Earlier this month, the pharmaceutical company Pfizer announced that it may ask the Food and Drug Administration to allow it to market the cholesterol-lowering statin Lipitor over the counter after its patent expires in November.
The FDA has not yet signaled whether it would approve Pfizer?s proposal. And some doctors have reacted coolly to the possibility of an OTC version of Lipitor.
But the development offers regulators ? and the public ? the chance to reconsider how patients access important medications. If done wisely, making more drugs available over the counter can generate significant savings for both consumers and the overall health care system.
Pharmaceutical companies face significant challenges in the immediate future. Patents for seven of the world?s top-selling brand-name drugs ? including the top two, Lipitor and Plavix ? are set to expire over the next 14 months.
About 4.3 million Americans take Lipitor. Each year, Pfizer takes in nearly $11 billion on sales of the drug. After the patent expires, those revenues will dwindle virtually overnight, as generic competitors enter the marketplace.
Consumers, of course, benefit from generic competition by paying lower prices. But the high prices of many patent-protected drugs generate the revenues needed to underwrite the massive research and development expenses that the inventors of brand-name drugs incur.
Bringing a single drug to market costs about $1.3 billion. For every drug that makes it to market, as many as 10,000 compounds fail. That?s why pharmaceutical firms invest more in RD than any other industry ? over $105,000 per sector employee as compared to an average of $10,000 across all U.S. manufacturing companies.?According to the National Bureau of Economic Research, U.S. drug companies invest about 17 percent of their sales in RD.
The looming patent expirations ? combined with those massive drug-development expenses ? is causing many pharmaceutical firms to shed jobs. Merck recently announced that it would eliminate 13,000 jobs by 2015. This year, the pharmaceutical sector as a whole has slashed 5,000 jobs ? on top of 54,000 in 2010 and 61,000 in 2009.
Those are substantial numbers. In 2009, the industry directly employed more than 674,000 U.S. workers. It supported an additional 3.4 million jobs indirectly. With patents expiring and ObamaCare set to assess millions of dollars in new taxes on drug firms, the sector could shrink even further. That would be bad news not just for workers but for all levels of government, as the industry pays about $85 billion annually in local, state, and federal taxes.
WASHINGTON (AP) ? As much as $60 billion in U.S. funds has been lost to waste and fraud in Iraq and Afghanistan over the past decade through lax oversight of contractors, poor planning and corruption, an independent panel investigating U.S. wartime spending estimates.
In its final report to Congress, the Commission on Wartime Contracting said the figure could grow as U.S. support for reconstruction projects and programs wanes, leaving both countries to bear the long-term costs of sustaining the schools, medical clinics, barracks, roads and power plants already built with American tax dollars.
Much of the waste and fraud could have been avoided with better planning and more aggressive oversight, the commission said. To avoid repeating the mistakes in Iraq and Afghanistan, government agencies should overhaul the way they award and manage contracts in war zones, the commission recommended.
The Associated Press obtained a copy of the commission's 240-page report in advance of its scheduled public release on Wednesday.
Created by Congress in 2008, the eight-member commission held more than two dozen hearings, interviewed hundreds of military and civilian officials and traveled multiple times to Iraq and Afghanistan. The panel's final report is the most comprehensive examination so far of the U.S. dependence on contractors and the government's ability to manage them in combat areas.
The commission said calculating the amount lost through waste and fraud is difficult because there is no commonly accepted methodology for doing so. Using information it has gathered over the past three years, however, the commission said at least $31 billion has been lost and the total could be as high as $60 billion. The commission called the estimate "conservative."
Overall, the commission said spending on contracts and grants to support U.S. operations in Iraq and Afghanistan is expected to exceed $206 billion by the end of the 2011 budget year. Based on its investigation, the commission said contracting waste in Afghanistan ranged from 10 percent to 20 percent of the $206 billion total. Fraud during the same period ran between 5 percent and 9 percent of the total, the report said.
Styled after the Truman Committee, which examined World War II spending six decades ago, the commission was vested with broad authority to examine military support contracts, reconstruction projects and private security companies. The law creating the commission also dictated that it would cease operating at the end of September 2011, even as the U.S. operations in Iraq and Afghanistan continue to be heavily supported by contractors.
The commission cited numerous examples of waste, including a $360 million U.S.-financed agricultural development program in Afghanistan. The effort began as a $60 million project in 2009 to distribute vouchers for wheat seed and fertilizer in drought-stricken areas of northern Afghanistan. The program expanded into the south and east. Soon the U.S. was spending a $1 million a day on the program, creating an environment ripe for waste and abuse, the commission said.
"Paying villagers for what they used to do voluntarily destroyed local initiatives and diverted project goods into Pakistan for resale," the commission said.
The Afghan insurgency's second largest funding source after the illegal drug trade is the diversion of money from U.S.-backed construction projects and transportation contracts, according to the commission. But the report does not say how much money has been funneled to the insurgency. The money typically is lost when insurgents and warlords threaten Afghan subcontractors with violence unless they pay for protection, according to the report.
The Associated Press reported this month that U.S. military authorities in Kabul have estimated that $360 million in U.S. tax dollars has ended up in the hands of people the American-led coalition has spent nearly a decade battling: the Taliban, criminals and power brokers with ties to both. The military said that only a small percentage of the $360 million has been taken by the Taliban and insurgent groups. Most of the money was lost to profiteering, bribery and extortion by criminals and power brokers.
The commission said the risk of greater waste looms as U.S. military forces withdraw from Iraq and Afghanistan. New infrastructure and security forces built up with largely with American money will be left behind. Buildings, roads and barracks must be maintained. Army and police forces must be equipped, fed and paid. If the Iraqi and Afghan governments cannot afford the bills, the investment could be squandered, according to the commission.
"These enduring costs risk wasting billions of dollars of American taxpayers' money ? possibly dwarfing the tens of billions in waste already incurred ? if funding from the Iraqi and Afghan governments or the international donor community cannot cover them," the report said.
The commission said that in early 2010 there were more contract employees ? 262,000 ? supporting the departments of Defense and State and the U.S. Agency for International Development in Iraq and Afghanistan than there were military and federal civilian personnel in both countries. But there are other considerations related to what the commission called an "unhealthy over-reliance" on the private sector for combat support and reconstruction work.
Contractors are handling duties that U.S. laws and regulations require government employees to perform, the commission said. For example, agencies often hire contractors to help evaluate or support its management of other contractors. This can create serious conflicts of interests, the report said, underscoring the need for a competent and well-staffed work force of government acquisition professionals. The reliance on contractors also overwhelms the government's ability to manage them, the commission said.
"Ten years of war in Iraq and Afghanistan have seen the United States using too many contractors for too many functions with too little forethought and control," the commission said.
The heavy dependence on contractors also obscures the full human cost of war, according to the commission. Contractor deaths and injuries largely have been undercounted and unpublicized by the U.S. government and the media, the report said.
Between October 2001 and July 2011, 2,429 contractors were reported by their employers to have been killed in Iraq and Afghanistan. There were 6,131 U.S. military fatalities during the same period.

NEW YORK (AP) ? Travelers across the country faced days of grief as thousands of flights were canceled Saturday because of Hurricane Irene.
Airlines scrapped more than 9,000 flights this weekend from North Carolina to Boston, grounding would-be travelers as Irene traveled up the East Coast. There were more than 3,900 cancellations on Saturday alone.
Millions of passengers will be affected by the time the storm finally dies as airlines work to accommodate travelers on very full flights. The biggest airlines, United Continental Holdings Inc. and Delta Air Lines Inc., each canceled thousands of flights.
All New York City-area airports closed to arriving flights at noon on Saturday, when the city's public transportation system shut down. By evening, the five major airports were closed.
Ronald Reagan Washington National Airport and Washington Dulles International Airport were both open Saturday afternoon, but most flights had been canceled.
The airports that will be most affected Sunday will be Newark Liberty International and New York's John F. Kennedy International, both with more than 1,200 cancellations, according to flight tracking service FlightAware. Boston's Logan and Washington Reagan were next in line.
Airlines have already canceled dozens of flights on Monday, but all the major U.S. carriers said they would wait to assess damage before canceling more. ExpressJet, which operates regional flights for United and Continental, has the most cancellations for Monday so far at 140.
Philadelphia International Airport closed altogether at 10:30 p.m. Saturday and planned to stay closed until at least 4 p.m. on Sunday.
The storm's timing was compounding problems. August is a busy month for air travel. The storm was expected to hit the Northeast overnight Saturday and early Sunday, which is the busiest time of the week to fly. Sunday is when many vacationers return from trips and many business travelers leave.
Airlines wouldn't say how many passengers would be affected by the hurricane, but the numbers will likely reach into the millions. That's because so many flights, both domestic and international, make connections through major East Coast hub airports. Even passengers not flying anywhere near the East Coast could be delayed for days as airlines work to get planes and crews back into position.
New York City experienced an unprecedented shutdown of its transit operations, the nation's biggest system of subways, buses and commuter rails ahead of Hurricane Irene. It was unclear when the system will run again. City subways alone carry about 5 million passengers on an average weekday.
Train and bus service was cut back elsewhere. Greyhound suspended service between Richmond, Va., and Boston for the weekend. Amtrak reduced its Northeast schedule Saturday and canceled all trains from Washington to Boston on Sunday.
Amtrak has five main routes throughout the Northeast, each serving multiple cities, as well as regional service in Virginia. Several long-distance trains to spots like Chicago and Montreal were affected as well.
Public transportation around New Jersey came to a halt, as trains stopped running and incoming flights were suspended at the New York area airports, including Newark Liberty.
Maryland transportation officials closed the Chesapeake Bay bridge when wind gusts reached 82 mph and suspended transit service Saturday evening. The bridge connects Annapolis and the rest of Maryland to the Eastern Shore.
Hurricane-force winds arrived near Jacksonville, N.C., at dawn. Around 7:30 a.m. EDT, the center of the storm ? estimated to be about 500 miles wide ? passed over North Carolina's Outer Banks. The hurricane's vast reach traced the East Coast from Myrtle Beach, S.C., to just below Cape Cod. Tropical storm conditions battered Virginia, Maryland and Delaware, with the worst yet to come.
Airlines were most recently hit with a natural disaster last winter, when they canceled thousands of flights ahead of a pair of massive snowstorms. The storms in December and February led to more than 10,000 cancellations over several days and left many thousands of people stranded at airports.
Airlines have been cutting flights over the last year, resulting in planes flying full or nearly so, in an effort to be more efficient. That makes it harder for stranded passengers to find empty seats on new flights once the weather gets better. Airlines waived ticket-change fees for most East Coast travelers affected by the storm. Some pushed off the $150 penalties for as much as a week to encourage travelers to make new arrangements.
American Airlines spokeswoman Andrea Huguely said the airline canceled some flights for Monday. The airline, owned by AMR Corp., is aiming to resume flights in North Carolina and Virginia after noon Sunday. She said it wasn't yet clear when flights in and out of New York would resume.
"The one thing about a hurricane is that you can prepare for it and you just have to adapt your plan based on how the storm travels," Huguely said. "It's basically an educated guessing game."

Prime Minister Recep Tayyip Erdogan was scheduled to announce the decision formally later Sunday when he hosts religious leaders and the heads of about 160 minority trusts, at a fast-breaking dinner for the holy Muslim month of Ramadan, officials said.
The properties include former hospital, orphanage or school buildings and cemeteries. Their return is a key European Union demand and a series of court cases has also been filed against primarily Muslim Turkey at the European Court of Human Rights. Last year, the court ordered Turkey to return an orphanage to the Greek Orthodox Patriarchate.
Some properties were seized when they fell into disuse over the years. Others were confiscated after 1974 when Turkey ruled that non-Muslim trusts could not own new property in addition to those that were already registered in their names in 1936. Mr Unai Aberasturi, a jurist at the University of the Basque Country (UPV/EHU), has made a study of what the Spanish Statute Law on Data Protection (LOPD in its Spanish acronym) says about this, drawing the conclusion that, normally, this is left open to interpretation. He warns of the judicial insecurity in this respect and stresses the need for concrete regulations on health data protection. His thesis is entitled The principles of data protection applied to health care.
The researcher undertook a joint study of health regulations and data protection. With the latter, he focused on the content of the LOPD norms bearing on the health sector, including most of those under its second section (principles of data protection) and third (rights of individuals). Mr Aberasturi pointed out that the LOPD provides special protection for health data (article 7), by which a stricter safeguarding scheme is set out.
Quality and consent
Within this regime, the so-called principles of quality should be respected although, as health data is involved, the thesis explains that compliance therewith has to be studied case by case. The principles of quality are, concretely, the principle of end purpose, of pertinence and of veracity. According to the principle of end purpose, the data cannot be used for a purpose that is different from that of the gathering. Mr Aberasturi explains that, of all the possible motives for data gathering in the health sector (health care, research, statistics, and so on), the purpose for each gathering must be specified. As regards the principle of pertinence, more data than is sufficient and pertinent should not be gathered. In order to protect the health of patients, medical records and documentation must be accessible to health personnel (health regulations require this), as well as be easy to deal with. Look, last year's Traktor Kontrol S4 was pretty damn awesome but, at $1,000 and about the size of your average Williamsburg DJ, it wasn't exactly for everyone. That's where the S2 comes in. Instead of four channels, it's a "2+1" setup that supplements the two standard audio ones with a dedicated sample channel. You still get the same high-resolution jog wheels and integrated 24bit / 96kHz sound card, but Native Instruments managed to shave a few inches, pounds and bucks off its predecessor. At $669 it's much more affordable -- still not exactly an impulse purchase, but within the range of most serious hobbyists.
